Most insurance will not cover cosmetic or elective services. With our plan, all services receive a 10% to 20% discount.

Pre-Existing Conditions Covered
Pre-existing conditions have no effect on your savings with our plan.
PROGRAM GUIDELINES
- Dual Plan is for Parent/Child or Spouse
- Cannot be used in conjunction with another dental plan
- Non-refundable
- No refunds on premiums will be issued at any time if participant decides to utilize dental plan.
- Patient’s portion of bill is due day of service.
EXCLUSIONS & LIMITATIONS
- Services for injuries under workman’s comp.
- Treatment which, in sole opinion of the treating dentist or doctor, lies outside the realm of their capability
- Referrals to specialists
- Hospitalization or hospital charges of any kind
- Costs of dental which are covered under automobile medical
